    Highlights

    Community Physician Network is seeking a Nurse Practitioner or Physician Assistant , to work alongside our Pulmonary Hypertension team at Community Heart and Vascular Hospital in Indianapolis, IN.

    • 4-10 hour shifts weekly, including in office and virtual home visits. Hours may vary based on patient volume/needs

    • Opportunity to practice in the Pulmonary Hypertension clinic, and inpatient hospital rounding.

    Qualifications

    • Experience practicing as a licensed Nurse Practitioner or Physician Assistant is required .

    • Pulmonary Hypertension, Heart Failure, or Palliative Care Experience is strongly preferred.

    • Must have excellent communication, critical thinking, and planning skills

    • Must be hard working, proactive, and dedicated to delivering high-quality patient care

    • Must be comfortable assessing patients independently

    Licensure & Certifications

    Educational background consistent with requirements for licensure and certification within the state of Indiana
