    Position Details:

    • Location : Farmington, CT. This is a hybrid position

    • Department : Care Coordination

    • Schedule : Full time, 40 hours/weekly, Monday through Friday, hours between 8:00AM - 5:00PM

    Primary Responsibilities:

    • Receive referrals or requests to provide support for members' health or psychosocial needs

    • Validate appropriateness of referral or follow relevant guidelines (e.g. protocols, procedures, job aids) to provide initial response to referral

    • Review specific diagnosis and member case files to understand reason for referral and identify critical data or information

    • Leverage understanding of relevant medical terminology to evaluate and review member case files and program applicability

    • Collaborate assessment efforts with multi/interdisciplinary team members to identify member needs and team member roles

    • Identify gaps in member's current state health or psychosocial needs and opportunities to provide assistance to members

    • Pursue additional information as needed to supplement gaps in available information (e.g. contact providers)

    • Review benefits information to determine what resources may be covered, whether additional resources should be identified and support case management process

    • Interacts with members base on their individual perspectives needs and desires (e.g. culture perspective, stage of duress, cognitive impairment)

    • Analyze data and information obtained, identifying root causes or underlying issues that may create barriers for members to achieving plan of care

    • Document assessment findings following organizational standards and best practices

    Required Qualifications:

    • Bachelors Degree in Social Work

    • 3+ years in acute, primary or hospital setting

    • 2+ years of experience of social work

    • Solid Microsoft Office Suite (Excel, and Word) experience

    • Basic computer knowledge and aptitude

    • Reliable transportation

    Preferred Qualification:

    • Masters Degree in Social Work

    • Experience with EMR systems (EPIC)

    • Experience in an ambulatory, physician practice setting, or health insurance environment

    Connecticut Residents Only: The salary range for this role is $58,300 to $114,300 annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
