Risk Stratification Nurse Coordinator - RN

The Chattanooga Heart Institute offers the most comprehensive cardiac care available in this region. Founded in 1976 we now have six offices in Tennessee and one in Georgia. Our practice is made up of 29 board-certified cardiologists and four cardiothoracic surgeons. We merged with CHI Memorial Health Care System in 2011 which is part of CommonSpirit Health. Responsibilities

Job Summary:

Coordinate the most effective action to assist patients with medication and patient assistance.

Essential Responsibilities:

• Communicate timely with outside offices and providers who are requesting risk stratification for our mutual patients.

• Triage patient appropriately per periprocedural cardiac risk evaluation algorithm.

• Review and complete appropriate patient documentation for periprocedural cardiac risk evaluation following algorithm approved by physician group.

• Coordinate as needed with clinical staff member assigned to Periprocedural Antithrombotic Management form.

• Assist PA/FMLA RN when assigned by manager.

Qualifications

Education:

• Graduate from an accredited RN program with one year in a clinical nursing setting.

License/Certification:

• Valid RN license in the state of Tennessee

• BLS Certification

Qualifications:

• Knowledge of EHR systems.

• Excellent written and verbal communication skills.

• Cardiology experience a plus.

Pay Range

$26.38 - $38.26 /hour
