    RN Case Manager Latham

    EDDY SENIOR CARE (full time days - no weekends, no on call)

    1) FOCUS ON MANAGING PATIENT CARE , not on the # of visits you do a day

    2) Work closely with an interdisciplinary team, including MDs and NPs, focused on prevention and achieving the best outcomes-long term relationships with patients/families focused on prevention vs reacting to episodes/crises

    Eddy SeniorCare is an exciting, innovative model of care for seniors, located at 385 Watervliet-Shaker Road, Latham. Our Program of All-Inclusive Care for the Elderly is located within Provincial House. We provide a team-based model of care including a physician, RNs, LPNs, Home Health Aides, physical/occupation/speech therapists, social workers, transportation, a day center team, and more! Currently most of our patients reside at the Sisters of St. Joseph; we also care for patients in the surrounding community.

    Primary responsibilities: include case managing an assigned caseload of participants, completing assessments, providing treatments, orienting LPNs and HHAs to the participant's plan of care and overseeing LPN and Aide care. Collaborates closely with the interdisciplinary care team.

    Work schedule: Monday to Friday, 8am to 4:30pm

    Requirements:

    NYS RN Licensure.

    Experience in home health or a long term care facility preferred but not required.

    Must possess a passion for caring for seniors and demonstrate a respectful, caring, professional demeanor.
