Our Queen's Clinically Integrated Physician Network team is seeking a compassionate and experienced Social Worker to join our team and make a meaningful difference in the lives of our clients.

    As a Social Worker, you will be a vital part of our team, working to provide support, counseling, and resources to individuals facing various challenges. You work will be instrumental in helping our clients overcome obstacles and lead healthier, more fulfilling lives.

    TYPICAL PHYSICAL DEMANDS

    • Essential: seeing, hearing, speaking, static gripping of an object for prolonged periods.

    • Frequent: standing, sitting, walking, lifting and carrying usual weight of 2 pounds, reaching above, at and below shoulder level, repetitive arm/hand motions.

    • Occasional: stooping/bending, kneeling, climbing stairs, walking on uneven ground, squatting, twisting body, finger dexterity, frequent gripping of an object.

    • Operates computer, telephone, copier, facsimile, cellular, pager, and other office type equipment.

    TYPICAL WORKING CONDITIONS

    • Not substantially subjected to adverse environmental conditions.

    • Exposed to communicable diseases and bodily fluids.

    • May be subjected to emotionally charged situations which may compromise physical safety.

    • Work involves off-site activities, including home or community outreach visits to patients.

    • Must have access to motor vehicle.

    MINIMUM QUALIFICATIONS:

    • Master's degree in Social Work

    • Current Hawaii State license as a Licensed Social worker (LSW) or Licensed Clinical Social Woker (LCSW) LCSW preferred

    • Valid U.S. driver's license required in accordance with Hawaii sate lawa rules and regulations

    • Driver's abstract or evidence of good driving record may be required based on area of assignment. Current BLS/HCP certification.

    • One (1) year experience in an acute care facility highly preferred

    • Experience to demonstrate knowledge of community resources throughout the state preferred. Previous geriatrics and/or mental health and substance use disorder training preferred.
