    JOB DESCRIPTION

    Job Title:Therapeutic Direct Support Professional (TDSP)

    Job Objective:To ensure the successful implementation of therapeutic services and treatment plans for high risk individuals.

    Reports To:Residential House Manager

    Status:Non-Exempt

    ESSENTIAL JOB FUNCTIONS

    1.Provides in-person support to high-risk individuals receiving services within the Broader Horizons Horne Rd treatment program.

    2.1:1 and/or group support and intervention which includes:

    a.Proactive behavior modification strategies

    b.Crisis de-escalation techniques, as outlined in the individuals' treatment plans and

    c.The program's policies and procedures.

    3.Assist individuals in completing approved program activities in the home and in the community.

    4.Intervene as needed when problem behaviors are exhibited. Intervening and addressing concerns and supporting the individual through difficult and challenging situations as outlined in the individuals' treatment plans.

    5.Ensures therapeutic skills are implemented during treatment and other programmatic approved activities and events.

    6.Works collaboratively with House Managers, Program Manager, other clinical treatment team members and management to ensure all treatment plans, which include Risk Management Plans, Behavior Plans and the Individuals' Treatment plans are being implemented, as well as meeting the needs of the individuals clinically and programmatically.

    7.Maintains logs of behavioral progress and concerns, relays information to the team members, supervisors and other staff as required. Therapeutic Direct Support Professionals report for weekly clinical consultation, as well as attend treatment team meetings and wrap around team meetings on a monthly basis or as needed.

    8.Individuals in this treatment program may receive services throughout the lifespan. When necessary due to medical or general health needs, the Therapeutic Direct Support Professional will assist in providing personal care.

    9.Employee must be able to fulfill the physical job demands for this position which could include lifting an individual supported within the program and/or assisting with a wheelchair or other mechanical device. See attached "physical job demands."

    10.Attend any and all continuing education/training as required.

    ~~~ limited situations Therapeutic Direct Support Professionals may be hired to provide support to high-risk individuals receiving services in other programs or residential homes.

    12.Other duties as assigned.

    JOB STANDARDS

    -Regular and reliable in-person attendance is required.

    -Ability to constructively participate in a treatment team environment for high-risk individuals with disabilities that may include; development/intellectual, traumatic brain injuries, mobility, and mental health/psychiatric impairments.

    -Ongoing training and/or continuing education as determined by the program leadership.

    -Ability to complete objective and accurate documentation. Ability to communicate effectively both orally and in writing. Ability to gather, organize and report information.

    -Ability to follow program guidelines and maintain strict professional boundaries with individuals receiving treatment.

    -The ability to pass medication under He-M 1201 when required.

    -Ability to follow procedures and maintain strict confidentiality.

    -Successful completion of all Agency and regulation-based background checks and pre-employment physical capacity screening.
